Primary biliary cholangitis and enthesitis

ABSTRACT

Background: Primary biliary cholangitis is a prototypical autoimmune disease characterized by progressive destruction of the intrahepatic ducts that leads to cholestasis and cirrhosis. The main cause of joint pain in primary biliary cholangitis is associated with overlapping autoimmune diseases (Sjögren’s syndrome and rheumatoid arthritis).
Clinical case: A female patient with primary biliary cholangitis (without associated autoimmune diseases) with clinical arthritis in the right knee and subclinical enthesopathy (musculoskeletal ultrasound) in the right patellar and left calcaneus. She was treated with ursodexosicolic acid, presenting remission of the symptoms.
Conclusions: Evaluation with musculoskeletal ultrasound is suggested in primary biliary cholangitis, a clinical entity where IL-17 plays an important role and has a crucial role in enthesitis in psoriatic arthritis and spondyloarthritis.
