Mixed hiatal hernia complicated with double gastric volvulus

ABSTRACT

Introduction: Gastric volvulus is a rare complication of hiatal hernia that can be life-threatening, defined as an abnormal rotation of part or all of the stomach around one of its axes.
Objective: To describe the case of a patient with a diagnosis of type III or mixed hiatal hernia with double gastric volvulus or combined with incarceration of the stomach.
Case report: A 70-year-old woman, admitted for chest pain and food intolerance is reported here. Contrast radiological study showed double gastric volvulus as a complication of mixed hiatal hernia. She underwent surgery urgently. Stomach release and reduction was performed, as well as closure of the hernial defect, and fundoplication.
Conclusions: The early diagnosis of gastric volvulus in stable patients with recent evolution of gastric obstruction allows the laparoscopic approach safely.
