Clinical outcomes of facet joint injection for the treatment of symptomatic lumbar disease

ABSTRACT
Introduction: chronic low back pain syndrome is a constellation of multiple symptoms most often associated with occupational disabilities. When compared to surgical intervention, facet infiltration in combination with physical therapy is a less invasive procedure which decreases overall risk of complications, thereby improving patient clinical outcomes.
Objective: evaluate the clinical effectiveness of facet infiltration accompanied by physical rehabilitation in patients with symptomatic lumbar degenerative disease.
Material and methods: observational study of routine clinical practice where patients diagnosed with symptomatic lumbar degenerative disease were evaluated using clinical and functional outcome scales (lumbar and radicular VAS, SF-36, Roland Morris and the Oswestry Disability Index [ODI]), before and after facet infiltration, in addition to 10 physical therapy sessions.
Results: a sample of 30 patients was obtained, and the clinical assessment was reported by preoperative ODI = 45.92 ± 17.00, postoperative ODI = 35.52 ± 4.56 (p = 0.008), preoperative lumbar VAS = 6.62 ± 0.29, postoperative lumbar VAS = 4.23 ± 0.47 (p = 0.0001), root preoperative root VAS = 6.38 ± 0.49, and postoperative root VAS = 4.08 ± 0.46 (p = 0.001).
Conclusion: 80% of patients with symptomatic lumbar disease who underwent facet infiltration and 10 physical therapy sessions showed clinical improvement at 3-month follow-up based on clinical and functional outcome scales (lumbar VAS, radicular VAS, and Oswestry Disability Index). For these patients surgical intervention was not necessary at 2-year follow-up.
