Utility of nuclear cells to hematoprogenitors collection in autologous transplantation of autoimmune diseases

ABSTRACT

Introduction: the CD34+ is useful for predict the optimal time to start hematopoietic progenitor cell (HPC) apheresis; however, it is a limited resource, and may not be available. In contrast, the quantification of total nucleated cells (CNT), is accessible and can guide the clinical practice of apheresis. Objective: to evaluate the usefulness of CNT to predict the optimal time to start apheresis of HPC in autologous transplants with autoimmune diseases. Material and methods: data from 256 patients with autoimmune disease with auto transplantation were included, and data were evaluated by recording CD34+ and CNT results before apheresis. The utility of CNT to predict the optimal time to start apheresis and obtain a CD34+ harvest > 2 × 106/kg was evaluated. Results: before apheresis, the median CNT was 1.4 k/µL; and CD34+ 59.5/µL. The ROC analysis of CNT before apheresis with CD34+ > 2 × 106/kg was 0.75 (CI 95% 0.67-0.84, p < 0.001), with a cut off 0.95 k/µL of CNT. Conclusions: the optimal time to predict to start HPC apheresis and harvest > 2 × 106/kg, in patients with autoimmune disease, is when obtaining > 0.95 k/µL.
