Reconstructive surgery with Latissimus dorsi in a patient with dermatofibrosarcoma

ABSTRACT

Dermatofibrosarcoma protuberans is a low frequency soft tissue tumor that occurs as a painless skin lesion with an insidious course. It affects people mainly between the ages of 20 and 59 years and is most common in the dermis or subcutaneous tissue of the trunk. Although its metastatic potential is low, it is aggressive locally with significant regional destruction; therefore, its management is mainly surgical. We present the case of a 56-year-old female patient with dermatofibrosarcoma protuberans in the anterior chest region. The patient had a history of tumors in the same area that had been resected on two previous occasions with histopathological reports of benignity. Physical examination revealed a fungoid tumor measuring 20 x 18 cm in the pectoral region, two nodules on the right shoulder and on the free edge of the pectoralis major, with no evidence of palpable lymph node involvement. Imaging showed voluminous solid and cystic lesions in the region, and the histopathologic report confirmed dermatofibrosarcoma protuberans. The patient underwent wide resection with a measured margin of 4 cm and coverage of the defect with a wide dorsal flap was performed. The discussion focuses on the diagnosis and management of this pathology, which should be aggressive to avoid local recurrence. We concluded that wide surgical resection is the main treatment for dermatofibrosarcoma protuberans, and long-term follow-up is necessary to detect recurrence.
