Baastrup´s disease: a frequent etiology of lower back pain

ABSTRACT
Introduction: low back pain is a frequent reason for consultation, its prevalence increases with age, it is estimated that 60 to 90% of the adult population will develop low back pain throughout their lives, becoming one of the main reasons for absenteeism. work in Mexico. Baastrup disease is a little-known cause of low back pain; it is caused by abnormal contact between the lumbar spinous processes during flexion-extension movements. It is attributed as one of the main causes of chronic low back pain as part of the degenerative process of the spine; however, due to the lack of diagnostic suspicion, directed treatment is rarely indicated, which triggers persistence of symptoms and therapeutic failure. Objective: to establish the prevalence of Baastrup disease in patients over 60 years of age with chronic low back pain and determine the findings in imaging studies, with the aim of increasing the diagnostic suspicion of said condition and including it in the list of possible causes of low back pain. at the Social Security Institute of the State of Mexico and Municipalities (ISSEMyM). Material and methods: cross-sectional and descriptive study, in which a total of 504 patients over 60 years of age with a diagnosis of chronic low back pain who underwent lumbar spine imaging studies were included. Results: the imaging studies of 504 patients were analyzed, of which in 267 cases (97 men and 170 women) changes compatible with Baastrup disease were found, thus establishing a prevalence of 52.97% in the studied population; however, in none of the cases mention this condition as a diagnostic conclusion. In all cases, a decrease in the interspinous space, reactive sclerosis and flattening of the interspinous contact surfaces were observed. Conclusions: Baastrup disease has a high prevalence in patients over 60 years of age with chronic low back pain. In advanced stages the diagnosis can be established through radiographic studies of the lumbar spine.REFERENCES
