Subtotal cholecystectomy, laparoscopic management for porcelain gallbladder. Case report

ABSTRACT
Introduction: the porcelain gallbladder represents a rare entity and it's associated with risk of malignancy. The treatment of choice is cholecystectomy, representing a surgical challenge given the degree of inflammation and fibrosis that this entity entails.
Clinical case: we present a 56-year-old female with a history of recurrent episodes of biliary colic. She presents abdominal pain in the right hypochondrium of 7 hours of evolution, VNS (verbal numeric scale) 7/10 and nausea. Surgical treatment with laparoscopic cholecystectomy was decided. A gallbladder with a high degree of fibrosis, pearly color and adhesions to the omentum was found, a subtotal cholecystectomy was decided. Intraoperative retrograde cholangiography shows bile leakage from cystic duct and stent placement is performed.
Conclusion: porcelain gallbladder is a histopathological condition in which there is calcification that infiltrates the gallbladder tissue. Found in 95% of cases of gallbladder lithiasis and represents special difficulty by limiting the field of vision due to its high degree of fibrosis. The prognosis depends on the degree of inflammation and the presence of malignancy. This entity entails a diagnostic and surgical challenge where the use of safe surgical alternatives for the patient becomes primordial, the technical knowledge and use of subtotal cholecystectomy is necessary for every surgeon who plans to perform laparoscopic cholecystectomy.
