David V procedure llega a México. Reporte del primer caso operado exitosamente

ABSTRACT

Finally, after nearly 25 years of existence, the David V procedure has been successfully performed for the first time in Mexico. Aortic valve sparing operations are surgical procedures to treat aortic root aneurysms. These basically comprise two modalities: David procedure (reimplantation), and Yacoub procedure (remodeling). Both of them work with a Dacron tube as a substitute for the dilated sinuses of Valsalva. For David V procedure, a Valsalva Graft® tube is used, which contains neo-sinuses of Valsalva. This makes it possible to reduce stress on the aortic valves at the time of closure.
We describe here the first case of aortic root aneurysm operated on successfully in a 27-year-old patient with frustrated Marfan syndrome, performing a David V procedure. The aneurysm measured by nuclear magnetic resonance 5.4 cm at the sinus level of Valsalva, 2.4 cm in the annulus, 3.3 cm at the sinotubular junction level. Preoperative transesophageal echocardiogram showed severe degree of aortic insufficiency, type I. A David V procedure was performed with a Vasalva Graft ® 30 mm, achieving reestablishment of aortic valve continence without residual insufficiency, while handling at the same time the concept of leaflet plication by means of of the caliper.
