Salivary aspartate aminotransferase biomolecule diagnostic of periodontitis as a bridge for multidisciplinary interaction

ABSTRACT

Introduction. An adequate multidisciplinary interaction between doctors and oral health professionals is necessary to reduce the repercussions of periodontal disease.
Objective. To determine whether salivary aspartate aminotransferase is the best diagnostic biomolecule for periodontitis compared to serum AST and salivary and serum Interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α).
Material and Method Methodology. Mixed methodology study composed of two substudies, the first experimental, comparative, cross-sectional, retrospective to determine the diagnostic capacity of salivary and serum AST, IL-6 and TNF-α in subjects without a diagnosis of periodontitis who attend the general medicine service and the second, experimental, comparative, longitudinal, to confirm the diagnostic capacity of salivary AST (ASTsa), before and after periodontal treatment in subjects who attend the oral health service. The diagnosis of periodontitis was determined using the Extent and Severity Index (IES).
Results. Only ASTsa levels were different between subjects without and with periodontitis (p ‹ 0.0005), correlated with the extent (r = 0.410, p ‹ 0.0005) and severity (r = 0.428, p ‹ 0.0005) of the IES, and presented a cut-off point of 16.0 IU/mL with high discriminative capacity (Area Under the Curve = 0.936, 95% CI, 0.864 - 1.000; p ‹ 0.001) In addition, after periodontal treatment, ASTsa levels decreased (116.9 ± 76.2. U/L vs 58.8 ± 47.0 U/L, p ‹ 0.001).
Conclusion. ASTsa is the best diagnostic biomolecule for periodontitis that could be used as a bridge for multidisciplinary interaction.
