ABSTRACT

Burns and hand injuries represent pathological entities with a high morbidity. There is a varying degree of hand involvement in as many as 70% of cases. High voltage burns in the upper extremity have a particularly poor prognosis for both the function and viability of the limb. Reconstruction of an arm with electrical burns continues to pose a challenge for plastic surgeons due to the limited availability of neighboring tissue, injury to deep tissues and the possibility of receptor blood vessel compromise in case of free flap usage. Perforator flaps have expanded the array of options available providing adequate cutaneous cover, with low morbidity in the donor area, allowing limb salvage in selected cases, sparing the patient the functional and psychosocial effects associated with limb amputation. We present the case of a 44-year-old male with significant forearm compromise, which was reconstructed with a free anterolateral thigh flap, with a favorable clinical evolution and whose arm was preserved.
