Correction of cosmetic units of the lower pole of the breast using triangular flaps

ABSTRACT
The goal of mastopexy includes lifting the nipple-areola complex (NAC); although, in some cases there is a displacement of the lower breast pole without affecting the NAC position. In this situation, correcting the lower breast pole is feasible without altering the NAC height as long as it is within acceptable limits. This work addresses the correction of the lower breast pole through the innovative use of triangular flaps in patients with a history of breast surgeries. We present two clinical cases demonstrating the effectiveness of this technique in correcting lower pole displacement, characterized by pseudoptosis, and bottoming out. The surgical approach involves resecting a triangular pattern with lateral and medial extensions, allowing significant skin resections, modification of the inframammary fold, and implant pocket changes without compromising the shape and height of the nipple-areola complex. The technique is valuable in revision surgeries where patients exhibit minimal ptosis and excess skin in the lower breast pole. These cases show successful outcomes, emphasizing the importance of evaluating anatomical dimensions carefully before the correction. We concluded that this technique offers a promising alternative for managing lower pole displacement in patients with prior breast surgeries, providing satisfactory results while preserving the natural aesthetics of the breast.REFERENCES
