Frequency and factors associated with mortality in pediatric patients admitted to the intensive care unit of a hospital in western Mexico

ABSTRACT
Objective: to estimate the frequency and risk factors associated with mortality in patients treated in a Pediatric Intensive Care Unit (PICU).
Material and methods: prospective cohort study of patients admitted to the PICU between September 2019 and September 2022. Analysis. The mortality rate per 100 admissions was estimated; to identify the factors associated with mortality, the OR and 95%CI were calculated using logistic regression.
Results: a total of 315 patients were studied; the median age was 60 months and 56.5% were male. The in-hospital mortality rate was 22.2 per 100 admissions (95% CI 17.9-27.1). Factors associated with mortality were hematologic malignancy (OR 4.2, 95%CI 2.1-8.3), cardiomyopathy (OR 23, 95%CI 2.3-228), requiring mechanical ventilation (OR 3.6, 95%CI 1.7-7.3), use of blood products (OR 2.4, 95%CI 1.3-4.3) and elevated serum lactate on admission (OR 1.2, 95%CI 1.1-1.4). While when the admission to the PICU was elective there was a lower risk of death (0.19, 95% CI 0.04-0.81).
Conclusions: in this study, a high mortality rate was found.The factors that were found to be associated with death, can help identify patients who require greater surveillance to try to reduce mortality.
