Implications of caring for grandchildren in vulnerable and critical contexts on the health of their grandmothers

ABSTRACT

Introduction. The current socio-economic changes have resulted in an increase in the responsibility of grandmothers for the care of their grandchildren, which may have a negative impact on their health. Objective. To describe the implications of caring for grandchildren in vulnerable critical contexts on the health of grandmothers in Cali, Colombia (communes 13 and 14). Methodology. A focused ethnographic methodology was employed, whereby 11 grandmothers caring for grandchildren aged 0 to 6 years participated for a period of 9 months. In addition to the collection of data through participant observation and semi-structured interviews, a field diary was maintained. Results. The findings reveal four cultural themes that exert a significant influence on the daily lives of these grandmothers. These are the grandmother’s personal history, the role of unpaid domestic work, the nature of the family context and the impact of responsibilities on the grandmother’s health. Conclusion. The care of grandchildren in critical or vulnerable environments has been identified as a significant social determinant of the health of the grandmothers in question. Poverty, insecurity and violence have permeated beyond the boundaries of the household, resulting in grandmothers being subjected to heightened levels of stress. The loss of their right to participate in household decision-making can result in a sense of marginalization. It is imperative that nursing addresses this issue by formulating targeted improvement plans, such as parenting education programs. Consequently, efficacious strategies can be implemented to promote the health and well-being of grandmother caregivers in these challenging environments.
