Case report on a 39.5-week corrected newborn with necrotizing enterocolitis and ileostomy

ABSTRACT

Introduction. Necrotizing enterocolitis is a real problem whose specific cause has yet to be identified or prevented in hospitalized premature newborns. However, it is a pathology with high rates of mortality and morbidity. The greater susceptibility of premature children has been attributed to the immaturity of their intestinal motility. Objective. To produce a case report focused on Dorothea E. Orem’s general theory on self-care concerning a patient with necrotizing enterocolitis IIIA, while maintaining appropriate stoma conditions along with the management of the micro and macro environment. Methodology. A qualitative method was followed. Data were obtained from clinical history and secondary sources as well as information found on scientific research platforms: Cochrane, Scielo, Medigraphic, Redalyc. Results. A weight gain of 80 g was obtained over the course of a one-week monitoring period with a score of 11 on the Infant Positioning Assessment Tool (IPAT). The mother was successfully trained in stoma care for home care, and effective breastfeeding was also achieved. Conclusions. Maintaining the stoma through appropriate care is essential for avoiding complications that could worsen the patient’s condition, as is managing the micro and macro environment in order to reduce stress in the hospitalized newborn patient.
