Postmastectomy breast reconstruction using the thoracoepigastric dermal fatty flap transposition technique with tissue expansion and definitive endoprosthesis

ABSTRACT

Introduction: Breast reconstruction is part of the comprehensive oncologic management of breast cancer for reducing the functional and aesthetic affections after mastectomy.
Objective: To describe the outcomes of postmastectomy breast reconstruction with the thoracoepigastric dermal fatty flap transposition technique using tissue expansion and definitive endoprosthesis.
Methods: A descriptive, prospective and longitudinal study was carried out. The universe consisted of patients with unilateral or bilateral modified radical mastectomy for breast cancer and who met the inclusion criteria. The sample consisted of twelve intentionally selected patients cared for at Hermanos Ameijeiras Clinical Surgical Hospital between October 2015 and June 2018.
Results: The age group of 41-50 years (58.3 %) predominated, together with the mixed race (75 %), hypertensive and diabetic patients (4.7 % and 3.9 %, respectively), patients mastectomized within the last 2-5 years (66.6 %), treated with hormones (100 %), and treated with chemotherapy (75 %). In 100 % of the cases, a volume of 100-200 cc was applied. The expansions were performed in three months. 100 % of the sample presented good aesthetic outcomes and two complications occurred.
Conclusions: The use of thoracoepigastric dermal fatty flap is a reconstructive alternative in patients operated on for breast cancer. It is an option from the current therapeutic arsenal since it allows to recover functional and aesthetic unity. It allows reconstruction of the defect with fewer sequelae and with better aesthetic outcomes, which also improves self-esteem and thus quality of life.
