Actinomycetoma in pregnancy: case report and therapeutic options

ABSTRACT

Mycetoma is a chronic granulomatous infection of the skin and subcutaneous tissue. In Mexico, 92% of cases are actinomycetoma, caused mainly by Nocardia brasilensis (78%). We report a 32-year-old in 28 weeks pregnant woman, presented with one year history of mycetoma on the dorsum of the left foot. She worsened during pregnancy. The biopsy confirmed actinomycetoma by the presence of Nocardia grains. She received an initial treatment with amoxicillin/clavulanic acid without improvement, presenting extension of the lesion. Cure was observed after delivery and six months of therapy with trimethoprim/sulfamethoxazole and rifampicin. Treatment of actinomycetoma during pregnancy is a challenge due to the rapid progression of the infection and limitation with prescription of antimicrobials.
