Temporomandibular joint replacement in sequelae of condylar trauma with custom joint prosthesis concomitant with orthognathic surgery: report of three cases

ABSTRACT
Introduction: condylar fractures secondary to direct or indirect mandibular trauma are common. Their treatment protocol is controversial between open-closed management. Although the results reported for open reduction and internal fixation (ORIF) of condylar fractures are superior, closed management is still chosen, with some developing class II dentofacial deformity and temporomandibular dysfunction secondary to aberrant condylar neoformation.
Clinical cases: we present three cases with condylar fracture treated by closed management that developed dentofacial deformity class II, aberrant condylar neoformation, pain and joint dysfunction. Due to disease progression, bilateral total temporomandibular joint (TMJ) replacement with custom prosthesis concomitant with bimaxillary orthognathic surgery (OS) with counterclockwise plane change was proposed as treatment.
Results: currently the patients have adequate joint function and maxillomandibular relationship, aesthethic facial profile and no pain.
Conclusions: in terminal stages the customized bilateral TMJ replacement concomitant with bimaxillary OS with counterclockwise plane change proves to be an effective and stable procedure during the follow-up time.
