Impact of cryoirrigation on CPK and PCR Biomarkers in oral surgery.

ABSTRACT

Introduction: using biomarkers such as C-Reactive Protein (CRP) and Creatine Kinase (CPK), the inflammatory response produced during the surgical procedure can be measured when lifting the flap and performing bone drilling. Objectives: compare the inflammatory response in the CRP and CPK biomarkers by irrigating the surgical bed with injectable solution at room temperature vs. cryoirrigation at 4 oC to reduce the symptoms associated with the postoperative inflammatory process. Materials and methods: this study is prospective, longitudinal, analytical, observational, quantitative, cases and control. It was carried out in patients who attended the Mexican Center in Stomatology Campus Morelia during the period 2023-2024. CRP and CPK levels were evaluated before and after surgery, with the purpose of demonstrating discrepancies in the inflammatory process. Results: a total of 39 surgeries were performed, 20 using irrigation at room temperature and 19 with injectable water at 4 oC. The initial and postoperative means of CPK and CRP, together with their standard deviations, were analyzed to apply the Student t test. With an alpha value of 0.05 and 37 degrees of freedom, a Student's t of 1.23 was obtained for CPK and 4.65 for PCR. A p = 0.2295 was observed for CPK and p = 0.0001 for PCR. The P value of CRP was significantly < 0.05, leading to the rejection of the null hypothesis, indicating a statistically significant difference between the means. Conclusions: the results obtained demonstrate that cryoirrigation mitigates the inflammatory response.
