The maneuvers to suspect and confirm elbow instability due to medial collateral ligament (MCL) injury are a) the Milking maneuver and b) the valgus stress test during movement; in the latter, the elbow is flexed from 20° to 30° with the forearm in a prone position, applying a valgus force, considering positive medial joint yawning greater than 1 mm in fluoroscopy.
