Lichen planus pigmentosus inversus: case report

ABSTRACT

Background: Inverse pigmented lichen planus is a rare variant of lichen planus consisting of grey-brown spots that affect non-photoexposed areas, mainly intertriginous areas. Clinical case: A 31-year-old female presented with hyperpigmented spots in pruritic folds that had been developing for 8 months. Dermoscopy revealed Whickham striae, a pigmented reticulum interspersed with blue-grey dots and globules. Histopathological study revealed focal parakeratotic hyperkeratosis, areas of hypergranulosis and regular acanthosis, with degeneration of the basal layer, lymphocytic infiltrate in the superficial dermis, and loss of melanic pigment. Laboratory studies were normal, and treatment was started with a high-potency steroid, with clinical response and resolution of the lesions. Conclusions: The inverse variant of pigmented lichen planus is an underdiagnosed entity that should be considered in the differential diagnosis of skin fold involvement.
