ABSTRACT

Introduction: Painful ophthalmoplegia is an unusual syndrome in daily practice, however, it includes a range of diagnostic possibilities that must be considered to guide complementary tests. Tolosa-Hunt syndrome is a rare cause of painful ophthalmoplegia, which is why its appearance is clinically interesting.
Objective: To describe the clinical and imaging manifestations of Tolosa-Hunt syndrome, as it is considered an illustrative case of unilateral painful ophthalmoplegia syndrome, with no involvement of other cranial nerves.
Case report: This is the case of a young adult who presented acute orbital pain and unilateral headache associated with ophthalmoplegia of the sixth cranial nerve without apparent cause. His diagnosis was decided as a Tolosa-Hunt syndrome, he responded satisfactorily to the treatment established with complete recovery of functionality.
Conclusions: A case of exclusion is described, which needed several diagnostic proposals. The importance of the clinical and radiological criteria proposed to date is established, and the effectiveness of first-line treatment is validated.
