Should replacement of the temporomandibular joint (TMJ) be the last resort for degenerative disease?

ABSTRACT

Degenerative diseases of the temporomandibular joint (TMJ), such as severe osteoarthritis, pose a significant clinical challenge due to their impact on patients' functionality and quality of life. This paper presents the clinical case of a 41-year-old female patient with bilateral TMJ osteoarthritis, who experienced progressive deterioration of mandibular function despite multiple conservative treatments and previous surgeries. Given the persistence of symptoms and advanced joint damage, bilateral joint replacement using custom-made alloplastic prostheses was performed. Postoperative results showed a significant improvement in mandibular function, with a considerable reduction in pain and increased mouth opening, leading to a substantial enhancement in the patient's quality of life. This case highlights the effectiveness of joint replacement in patients with severe TMJ damage, suggesting that the intervention could be beneficial not only in terminal stages but also in patients at high risk with clear disease progression. The importance of a comprehensive diagnostic approach and the need for early prediction of disease progression are discussed, which could enable more timely interventions based on personalized medicine.
