Warfarin resistance. Case report

ABSTRACT
Warfarin is the most widely used oral anticoagulant in the long-term prophylaxis of thromboembolic complications accompanying various diseases. Indications have expanded in recent years, as new clinical situations predisposing to thrombotic events have been detected. Due to the special characteristics: highly variable dosage in each individual, narrow margin between adequate and inadequate dosage, interactions with multiple drugs, interference of the effect due to the high consumption of vitamin K in the diet and the possibility of the appearance of hemorrhagic complications or recurrence of thrombosis. The use of the drug requires strict dosage control and continuous clinical and laboratory monitoring. An 81-year-old female patient, with a personal pathological history of arterial hypertension, heart failure and epilepsy, is admitted to the Angiology Department of the "Hospital Manuel Fajardo" for presenting pain and increase in volume of the left lower limb for four days. It was decided to start treatment with anticoagulant low molecular weight Heparin (Fraxiheparin) and warfarin (2 mg) and it was decided to increase the dose of warfarin because the expected response to treatment with the anticoagulant was not obtained.
