Urinary tract infection by Trichosporon asahii in a pediatric patient with aplastic anemia

ABSTRACT

Trichosporon asahii is a widely distributed yeast that can be part of the skin biota. It is considered a colonizing, opportunistic microorganism in immunocompromised patients. It can produce local or systemic infections; as well as infections associated with health care. The urinary tract is the site of infection with the highest incidence.
Clinical case: We present the case of a 13-year-old female, diagnosed with cerebral palsy, chronic malnutrition and aplastic anemia secondary to Epstein-Barr virus infection. She was admitted for pneumonia complicated by empyema, requiring endopleural catheter, vasopressor support and mechanical ventilation. She presented urinary changes, dysthermia, with elevated CRP 70 mg/l; two positive urine culture with T. asahii: ›100 000, ruling out infection at another level. She received treatment with fluconazole for 14 days with favorably evolution. The patient showed clinical improvement and a negative urine culture result.
