Displacement of a fragment of the lower third molar into the submandibular space and its surgical medical management.

ABSTRACT

Introduction: the displacement of third molars to deep planes is an infrequent complication and rarely reported in the literature. This type of complication can be accompanied from a biological point of view by severe tissue damage, psychological discomfort and certain medical-legal conditions. Objective: to describe a clinical case of displacement of a fragment of the lower third molar into the submandibular space and its medical-surgical management. Clinical case: a 24 years old male patient referred by a general dentist due to a transoperative complication in the surgical extraction of a lower left third molar with a five days evolution, during which time he underwent treatment with amoxicillin 500 mg and ibuprofen 600 mg. In this procedure, the distal corono-root fragment of the molar was displaced after odontosection. Clinically the patient presented limitation of oral opening, slight inflammation at the level of the left posterior mandibular body due to the surgical procedure and slight pain in the operated area. Conclusion: to be aware of the limitations as general dentists when carrying out a complete analysis of each of the patients and, if necessary, to refer patients with molars in complex positions to the specialist in oral and maxillofacial surgery.
