ABSTRACT

Introduction: cleft lip and palate is one of the congenital defects that often leaves major sequelae such as severe maxillary hypoplasia where only conventional orthognathic surgery is not the best treatment option. Distraction osteogenesis is an alternative with the implementation of the RED Device with the application of artificial intelligence (AI) in the design of guides, optimizing time and improving the preservation of anatomical structures. Objectives: to emphasize the importance of AI-assisted virtual planning in the design of a drilling guide. Clinical case: a 16-year-old patient with sequelae of cleft lip and palate is presented. The patient underwent distraction osteogenesis with virtual planning and the application of AI. In addition, a systematic review of the literature was conducted. Results: it was evident that the application of AI in the virtual planning of maxillary distraction osteogenesis with the use of the RED Device allowed a gradual advancement of the maxilla and controlled positioning of the pins. Conclusions: AI-assisted distraction osteogenesis improves surgical precision, optimizes outcomes, and minimizes complications, offering an effective and stable alternative for the treatment of severe maxillary hypoplasia in patients with cleft lip and palate.
