Spontaneous avulsion of the Achilles tendon enthesis due to chronic tendinopathy in a septuagenarian with Haglund deformity

The Aquiles tendon transmits the contraction force of the triceps surae to the calcaneus; the contraction and/or sudden stretching of the triceps surae in older patients can generate avulsion of the posterior tubercle of calcaneus due to poor bone quality or avulsion of the enthesis of the Achilles tendon (like the case presented), affecting it by chronic tendinosis due to the presence of Haglund's deformity. Calcaneal avulsion fractures represent 1.3 to 2.7% of all calcaneal fractures. Risk factors are age, chronic tendinopathy, diabetes mellitus, nephropathy, and use of drugs such as steroids or quinolones.
