ABSTRACT

We present the case of a 34-year-old male with gastroesophageal reflux disease who developed an acute extrapyramidal syndrome after receiving intravenous metoclopramide (10 mg) following a laparoscopic fundoplication. The patient exhibited symptoms including lingual dystonia, spasticity, akathisia, dysphagia, and dyspnea. Paraclinical studies, including electroencephalogram (EEG) and magnetic resonance imaging (MRI), yielded normal results. Treatment with diphenhydramine (50 mg) resulted in significant improvement in rigidity and other symptoms. The patient continued on diphenhydramine every eight hours and was discharged within 24 hours.
