McKeown esophagectomy, a treatment option for esophageal caustic stenosis

ABSTRACT

Introduction: caustic ingestion is a problem that requires multidisciplinary management given the associated morbidity and mortality. Esophageal stricture represents the main long-term complication of patients with second-and-third degree esophageal burns, however, most of these patient's don't require surgical intervention and only small number of them may require it. Case report: a 29-year-old male presents esophageal stenosis secondary to ingestion of caustic soda due to a suicide attempt, debuting with inability to feed orally and dependent on feeding through a previously performed jejunostomy. Endoscopy and esophagogastroduodenal series were performed with results compatible with esophageal stenosis, with delayed and unsuccessful endoscopic, so esophagectomy McKeown surgery is performed with gastric ascent. Conclusions: there is great controversy about the ideal or optimal technique for esophageal resection. Esophagectomy by thoracotomy is a widely used technique in the treatment of esophageal cancer, however it also represents a treatment option for benign lesions of the esophagus such as those caused by caustics, mainly in strictures refractory to endoscopic treatment, since it allows greater control of the structures and better management of anastomotic leaks.
