ABSTRACT

Introduction: teratomas are neoplasms derived from totipotent germ cells, capable of differentiating into tissues from the three germ layers: ectoderm, mesoderm, and endoderm. Although most are located in the gonads, less than 5% develop in extragonadal sites, with the greater omentum being an exceptionally rare location. This rarity makes them clinically significant due to their low incidence and distinctive characteristics. Case report: we report the case of a 49-year-old woman with a chronic abdominal mass. Imaging studies, including ultrasound and contrast-enhanced computed tomography, revealed a predominantly cystic, encapsulated mass without signs of invasion or malignancy. Tumor marker levels, such as CA-125 and alpha-fetoprotein, were within normal ranges. During exploratory laparotomy, a lesion was found adhered to the greater omentum, containing sebaceous material, hair, and teeth. Histopathological analysis confirmed a mature monodermal teratoma (struma ovarii). Complete surgical excision was performed, along with a left salpingo-oophorectomy due to an associated ovarian cyst. Conclusion: this case highlights the importance of an interdisciplinary approach, clinico-pathological correlation, and complete surgical excision as the definitive treatment for rare neoplasms.
