Reverse flow sural flap for skin coverage of a traumatic wound in the Achilles region

ABSTRACT
The Achilles region is a critical area of the lower limb for reconstructing a defect, where graft survival is uncertain, making flap use a good option. We analyzed the advantages and disadvantages of using a reverse-flow sural flap to cover foot skin defects. We present the case of a 30-year-old woman admitted due to a traumatic wound in the right Achilles region, resulting from a motorcycle accident. A reverse-flow sural flap was performed with a favorable evolution at one-month follow-up. The reverse-flow sural flap is an axial flap commonly used for such problems, providing long term arterial supply survival owing to its arterial supply, making it one of the most popular favored options. Its disadvantage is potential venous congestion, depending on the patient's factors and condition. We conclude that the reverse-flow sural flap is a viable option to treat these defects considering patient's clinical characteristics.
