PEEP goals in SIRA severe based on PaFi for the Mexican population based on the experience of the SARS-CoV-2 pandemic.

ABSTRACT
Introduction: within the damage induced by the ventilator, one of the most important mechanisms is the so-called "atelectrauma", where the collapse and reopening of alveoli occurs repetitively, mainly in areas where the function of the pulmonary surfactant is altered, something that is very frequent in patients with ARDS. To prevent its development, the use of positive end-expiratory pressure (PEEP) has been proposed by several authors.
Material and methods: patients admitted to intensive care for SARS-CoV-2, with clinical symptoms of acute respiratory failure, compatible with ARDS; in most cases, patients, despite symptomatic treatments, presented with a torpid evolution towards deterioration, requiring intubation; once in this critical situation, other treatment measures were added, and based on the results of the PaFi, a new table was developed.
Results: a new PEEP table based on the PaFi obtained as an alternative in patients with severe ARDS is proposed.
Conclusion: the PEEP/FiO2 table was useful in patients with severe ARDS.
