Management of open abdomen during pregnancy

ABSTRACT

In the humanity history, pregnancy and outcome are a challenge even today. Historically, until a century ago, a cesarean section was a very risky procedure for both, the mother and the fetus. If we add to this the possibility of an illness during gestation, the pregnancy, most of the time, could not continue and the results ended up being fatal. This has changed, however, in special occasions the decision that must be made, the doctors and the patient and family, apart from being difficult, does not guarantee a good result. Even today these situations prevail. In this clinical case, we are faced with a challenge. A young mother with a second trimester pregnancy and an additional intestinal perforation. Operating on her was the logical decision, but continuing the pregnancy with the complications presented, such as an open abdomen, secondary to the growth of the pregnant uterus, was quite a challenge. Because of this, we present our management method, which becomes an option to continue with a pregnancy to term and thus ensure the well-being of both and therefore for the family and society. It also becomes an option in the case of valuable products with patients who have to be operated on for an additional disease, and in those cases where all the means are not available, such as a second level, and similar complications occur during pregnancy, thus avoiding premature interruption of pregnancy, transfers, as well as their dramatic results. It is important to point out that it not only depends on the method and the doctors in contact with the patient, but it also constitutes a great team effort to obtain satisfactory results, as was the case in this case.
