Contrast-enhanced lung ultrasound (CELUS) in patients with SARS-CoV-2 pneumonia. A study of three cases

ABSTRACT
Introduction: in patients with SARS-CoV-2, different coagulation abnormalities have been demonstrated, as well as systemic microcirculatory disorders, which produce an increase in thrombotic events linked to poor prognosis. Contrast-enhanced lung ultrasound (CELUS) identifies embolic consolidations by demonstrating the absence of enhancement upon contrast administration.
Objective: demonstrate the absence of contrast enhancement in peripheral lung lesions of three COVID-19 pneumonia patients by using CELUS.
Materials and methods: Design: study of three cases. Settings: Intensive Care Unit of the General Hospital of Zone No. 4. Mexican Institute of Social Security, Monterrey, Nuevo León, Mexico. Patients: Three patients with positive RT-PCR for SARS-CoV-2 and presence of peripheral lesions identified by B Mode Ultrasound. Intervention: administration of 2.4 mL of SonoVue/Lumason™ intravenously followed by a bolus of 10 mL of saline solution. Contrast enhancement of peripheral lesions was observed continuously during 120 seconds. The mechanical index of the Ultrasound was set to 0.3 to minimize the risk of Microbubbles disruption.
Results: No contrast enhancement was observed in peripheral lesions during the complete CELUS examination of all three cases, which is highly suggestive of embolic lesions.
Conclusion: The use of CELUS in patients with COVID-19 pneumonia can help identify peripheral lung lesions resulting from microvascular infarction, which are difficult to visualize with other imaging methods.
