ABSTRACT
Introduction: At present there is not any ideal pharmacological agent that guarantees optimal analgosedation for colonoscopies. In this regard, there has been particular interest in the ketamine-propofol (ketofol) combination.Objective: To characterize analgosedation with ketofol (1:20) in continuous infusion for elective colonoscopy.
Method: An observational, descriptive and prospective study of a series of cases was performed in 106 patients. A solution obtained by associating 0.5 ml of ketamine (5%) and 50 ml of propofol (1%) was used. The ketamine-propofol ratio was 1:20. The bolus and infusion dosage was calculated taking propofol as a reference with a loading dose of 1.1 mg/kg, equivalent to a 2 μg/ml plasmatic concentration. Cardiorespiratory variables were studied during the pre, trans and postoperative period: mean arterial pressure, heart rate, peripheral oxygen saturation (SpO2), sedation level according to Ramsay score and adverse events.
Results: There was a tendency to cardiorespiratory stability at different moments of the procedure, only a slight decrease in mean arterial pressure during the intraoperative period compared to pre and postoperative. The Ramsay score reflected minimum sedation levels between II and III (6.60 and 93.39%) during the entire endoscopic procedure, with a maximum of all patients having a III score. Only six patients (5.66%) presented adverse events, pain being the most frequent.
Conclusions: Sedoanalgesia with ketofol (1:20) in continuous infusion represents a new and useful strategy.
