Intestinal Occlusion Associated with an Appendicular Ring Resolved by Laparoscopy

ABSTRACT

Introduction: We report a case of intestinal occlusion secondary to an appendicular ring, a very rare pathology. The diagnosis was made surgically, guided by abdominal computed tomography, which suggested intestinal occlusion without specifying the etiology.
Case report: A 73-year-old woman presented to the emergency department with abdominal pain and nausea up to vomiting, without peritoneal irritation or appendicular signs. Computed tomography revealed a swirl image at the mesogastric level, suggestive of internal hernia and intestinal loop distension.
Discussion: Diagnostic laparoscopy revealed an appendicular ring in the terminal ileum, along with an enlarged cecal appendix. An appendectomy was performed, and indocyanine green confirmed adequate vascularization of the affected intestinal loop.
Conclusion: This rare pathology is a surgical challenge and should be considered as a differential diagnosis once the most frequent causes of occlusion, such as adhesions, hernias, or tumors, are ruled out.
