Advanced wound closure: a new technique for the management of wound dehiscence in abdominoplasty

ABSTRACT
Abdominoplasty ranks among the most frequently performed aesthetic surgeries worldwide. However, wound dehiscence remains a challenging complication, and traditional treatments such as topical care and negative pressure wound therapy are sometimes insufficient. This work presents a modified wound management technique, advance wound closure, that combines tension-reducing sutures with negative pressure therapy for treating complex dehiscence following abdominoplasty. We treated a 43-year-old female who underwent conventional abdominoplasty with liposuction. On postoperative day five, she developed wound dehiscence measuring approximately 15 × 10 cm, with signs of infection. Initial management with topical antibiotics and negative pressure therapy failed to reduce the defect. A modified technique was used, combining anti-tension sutures with a layered negative pressure system, resulting in progressive and complete closure without recurrence. The patient showed a satisfactory aesthetic outcome during a three-month follow-up, without complications. Advance wound closure offers a promising solution for complex postoperative dehiscence in abdominoplasty patients, potentially avoiding grafts and improving overall outcomes.
