Correlation between preoperative values of parathormone and the weight and size of the parathyroid adenoma in patients with primary hyperparathyroidism

ABSTRACT
Introduction: primary hyperparathyroidism is caused in 80-85% of cases by autonomous PTH production from a single parathyroid adenoma. The variability in the number and location of the parathyroid glands, as well as their size and the morphological similarity between healthy and pathological tissue, makes parathyroid surgery highly challenging. The use of preoperative serum markers such as PTH to estimate adenoma size and volume can aid surgical planning and thereby reduce the level of complexity of the procedure.
Objective: to determine if there is a correlation between preoperative serum PTH level and the size and weight of a parathyroid adenoma in patients with primary hyperparathyroidism.
Material and methods: retrospective study that included patients with a primary hyperparathyroidism diagnosis, who underwent a selective parathyroidectomy in Hospital Angeles Pedregal in Mexico City during the period between 2018 and 2024.
Results: a total of 89 patients, 17 men and 72 women, were included, with a median age of 58 years. Spearman's rho is used to calculate the correlation between perioperative serum PTH and the size and weight of the parathyroid adenoma. A positive correlation was obtained, with a correlation coefficient of 0.246 for size and 0.255 for weight.
Conclusion: the study showed a positive correlation between preoperative serum PTH levels and the size and weight of the parathyroid adenoma.
