Patient management of the prosthetic heart valve patient

ABSTRACT
The management of the patient with cardio-valvular-prosthesis is extremely risky, because they are susceptible of endocarditis, and hemorrhage subsequent to dental procedures that involve or imply bleeding. The objective was to determine the management of patients with cardio-valvular-prosthesis at the Hospital de Cardiología Centro Médico Nacional Siglo XXI. The study was done in 23 patients (males and females) from 3 to 78 years old, who had valvular-protesis, and required dental treatment. The results showed that the stomachological management in those patients required premedication with antibiotic therapy in order to avoid bacterial endocarditis; besides that is was needed to suspend the anticoagulant, and the use of hemostatic in order to prevent hemorrhage. Despite of the measures taken, three patients presented continuous bleeding, bleeding which ceased in two of them when the anticoagulant was suspended, and when a freez fresh plasma was provided. In conclusion it is sugested. that the stomachological attention of patients with cardio-valvular-protesis must be performed in a hospital with a multidisciplinary medical approach, with the needed resources to protect the patient’s life.
