Correlation between the optic nerve sheath diameter and the severity of lesions in geriatric patients with traumatic brain injury

ABSTRACT

Introduction: traumatic brain injury (TBI) is a widespread health issue in the geriatric population, and traumatic brain injury (TBI) has emerged as a new epidemic in this age group, being associated with worse clinical outcomes.
Materials and Methods: this was a prospective, observational, cross-sectional, and analytical study conducted on geriatric patients of both sexes who presented with severe traumatic brain injury (TBI). The measurement of the optic nerve sheath diameter (ONSD) was performed by two trained investigators with more than 30 years of combined experience.
Results: a total of 46 patients were included, of whom 31% (n=14) were male. The median age of the patients was 74.5 years (range: 24-85). Regarding the temporal evolution, a median of 9 days (range: 4-24) was observed, while the median length of hospital stay was 7.0 days (range: 3-13). The most frequent comorbidities were arterial hypertension (17%) and diabetes (9%). The group with pulmonary contusion showed higher mortality with 14 cases (30.4%). The utilized logistic regression model demonstrated statistical significance with a p-value (Omnibus) less than 0.002. This indicates that the model as a whole is highly significant and supports the association between the analyzed variables and ONSD.
Conclusions: ONSD can provide valuable information in the assessment and monitoring of geriatric patients with severe TBI, particularly in those with thoracic trauma.
