Usefulness of serum albumin levels in the diagnosis of abdominal sepsis secondary to complicated appendicitis

ABSTRACT
Objective: to determine the diagnostic capacity of serum albumin levels for identifying pediatric patients with complicated appendicitis.
Material and methods: a diagnostic test study was conducted on 210 patients aged 3 to 17 years with a surgical diagnosis of acute appendicitis from three hospitals in Puebla, Mexico, between 2023 and 2024. Clinical and laboratory data were compared between patients with complicated appendicitis (n = 125) and those with simple appendicitis (n = 85).
Results: albumin levels were lower in patients with complicated appendicitis (3.87 g/dL) than in those with simple appendicitis (4.35 g/dL), p < 0.001. The cutoff point of 3.8 g/dL showed a sensitivity of 60%, specificity of 95%, positive predictive value of 80%, and negative predictive value of 49%.
Conclusions: serum albumin levels have limited utility in identifying patients with complicated appendicitis.
