Visceral leishmaniasis in a pediatric patient: diagnostic challenges in Mexico

ABSTRACT
Introduction: we present the case of a patient diagnosed with visceral leishmaniasis treated at a hospital in Puebla, Mexico, to highlight the diagnostic and therapeutic challenges.
Case description: a one-year-eleven-month-old girl from a municipality in Puebla was admitted with persistent fever, hepatosplenomegaly, and pancytopenia. The diagnosis was established by bone marrow aspiration and culture, complemented by histopathological and biochemical studies, which confirmed the presence of Leishmania sp. and showed marked hypergammaglobulinemia. Treatment with meglumine antimoniate (20 mg/kg/day for 20 days) was prescribed, resulting in complete clinical and hematological resolution.
Conclusions: this case, documented in an area with recorded transmission of the parasite, underscores the need to maintain a high index of clinical suspicion in pediatric patients with prolonged fever and visceromegaly.
