ABSTRACT

We present the clinical case of a kidney transplant recipient from a deceased donor, in whom protocol biopsies revealed intratubular casts as a possible finding related to acute tubular injury associated with tacrolimus use. However, due to the presence of these casts, monoclonal gammopathies must be ruled out as a potential cause of acute dysfunction.
