ABSTRACT
Clear cell carcinoma (CCC) is characterized by paradoxical clinical presentations and rare metastatic spread. Sarcomatous differentiation is present in 5% of cases. Bone and lung metastases are common, but cardiac extension is extremely rare. This is due to CCC extending via the renal vein and inferior vena cava in 5-15% of cases, affecting the right atrium in 1% of patients, or presence of a primary tumor that metastasizes directly to heart, occurring in 10-20% of patients who die from disseminated CCC. Metastases without vena cava involvement or without evidence of disseminated disease is extremely rare.
