ABSTRACT

Introduction: advances in endodontics, including new techniques, materials, and CBCT imaging, have improved treatment accuracy and success. However, pronounced root curvatures remain a challenge, making root canal instrumentation and obturation difficult. Tools such as endodontic microsurgery and the use of more flexible instruments allow for more conservative and effective approaches. Objective: to analyze variations in mesial root curvature of mandibular first molars using CBCT in patients at the Orthodontic Clinic of the Autonomous University of Nayarit, in order to optimize endodontic treatment planning. Material and methods: a descriptive study was conducted from 1,800 CBCT examinations, selecting 167 scans of men and women for analysis of the degree of curvature in lower first molars, with a confidence level of 99%. Results: the evaluation showed an average angulation of approximately 29.8o in both sexes, with a predominance of moderate curvature. No statistically significant differences were detected between sides or genders. Conclusion: CBCT imaging facilitates accurate three-dimensional assessment of buccolingual curvature in the mesial roots of mandibular molars. The prevalence of moderate curvatures represents a challenge for endodontic treatment. Therefore, it is essential for the practitioner to have a thorough understanding of dental morphology and appropriate use of diagnostic tools such as radiographs to ensure clinical success.
