Relationship between stage IV periodontitis and cardiovascular diseases.

ABSTRACT
Cardiovascular diseases (CVD) and periodontitis are highly prevalent non-communicable diseases that represent a major global public health concern. Both conditions share common risk factors and pathophysiological mechanisms associated with systemic inflammation. In this context, periodontitis has been proposed to contribute to the development and progression of CVD through the dissemination of periodontal pathogens and inflammatory mediators into the systemic circulation. Stage IV periodontitis represents the most advanced form of periodontal disease, characterized by severe destruction of the supporting tissues, tooth loss, and significant functional impairment, requiring complex therapeutic management. Due to its high inflammatory burden, this condition may exert a greater systemic impact, particularly on the cardiovascular system. The aim of this review is to analyze the available scientific evidence regarding the association between Stage IV periodontitis and CVD, as well as the biological mechanisms involved. Current evidence suggests that systemic inflammation, bacterial dissemination, and the host immune response play a relevant role in this relationship; however, further longitudinal studies and controlled clinical trials are needed to better establish its causal nature.
