ABSTRACT

Blackwater fever is a serious complication of malaria characterized by massive intravascular haemolysis, which leads to haemoglobinuria, anaemia, and, in some cases, acute kidney injury. Its pathophysiology is not well understood. We present two cases of blackwater fever in two children with Plasmodium falciparum malaria treated with artemisinin-based therapies. We emphasize the importance of diagnosing blackwater fever as a complication in children with malaria, making the differential diagnosis with other causes of haemoglobinuria.
