Risk factors associated with Clostridioides difficile infection in pediatric patients

ABSTRACT

Introduction: Clostridioides difficile infection (CDI) is a growing cause of pediatric morbidity, especially in children with chronic comorbidities. Objective: to identify risk factors, prevalence, and morbidity and mortality associated with CDI in the pediatric population of Centenario Hospital Miguel Hidalgo. Material and methods: observational, analytical and retrospective case-control study (n = 154) in patients under 18 years of age with acute diarrhea evaluated by multiplexed PCR (FilmArray ®) between 2021 and 2025. Results: 77 cases and 77 controls were included. No significant differences were found in prior antibiotic use (p = 0.316). Oncological comorbidities (48.8 vs. 21.4%) and heart disease (17.1 vs. 4.8%) were significantly more frequent in cases (p = 0.037). A positive GDH antigen was associated with the presence of toxins (OR = 15.22, p = 0.0018). Conclusions: CDI is mainly associated with specific comorbidities (oncological and cardiac).
