Irritant Contact Dermatitis Secondary to Capsaicin Use in an Oncology Patient

ABSTRACT

Capsaicin is a natural alkaloid derived from Capsicum peppers used transdermally as part of the therapeutic arsenal for neuropathic pain, particularly in second- or third-line treatment. Although its use is generally safe, local adverse effects such as burning and erythema are considered expected reactions. We present the case of a 64-year-old female with synchronous double malignancy (pancreatic carcinoma and colon adenocarcinoma), who developed grade 3 palmar-plantar neurotoxicity after eleven cycles of oxaliplatin. As part of symptomatic management, capsaicin patches were applied to both palms. Subsequently, the patient developed irritant contact dermatitis, identified as an adverse reaction to treatment.
