Trichoscopic findings in a patient with dermatomyositis: a case report

ABSTRACT
Scalp involvement in dermatomyositis (DM) is common and
may present with diffuse erythematous scaly lesions, cutaneous
atrophy, and non-scarring alopecia, with pruritus being
a characteristic symptom. As these manifestations can
be severe, treatment-refractory, and negatively affect quality
of life, an accurate diagnostic approach is essential. In
this context, trichoscopy emerges as a non-invasive tool
that allows the identification of patterns suggestive of DM
scalp involvement, including enlarged and tortuous vessels,
megacapillaries, and interfollicular scaling.
We report the case of a 20-year-old male with hypomyopathic
juvenile dermatomyositis receiving immunosuppressive
therapy, who presented with intense pruritus and
persistent scalp erythema. Trichoscopy revealed an erythematous
background with enlarged and tortuous vessels,
megacapillaries, and whitish crusts. Given the suspicion
of active disease, a scalp biopsy was performed, confirming
DM and allowing optimization of immunosuppressive
treatment.
